When you visit Woodhaven Dental for the first time, you will be asked to review, complete, and sign the following forms.
In order to save you time, you may fill out the forms beforehand and bring them with you to your first appointment. Copies are available in our office if needed.
We pride ourselves on treating our patients with the utmost respect, honesty, and compassion. We promise to give you an honest opinion and educate you on the recommended treatment options. During the first visit, you will have time to get to know your doctor and we will obtain your past medical history. We will take the time to get to know you, as well as learn your expectations from us. After finishing the first dental exam, we will discuss the next steps in your dental journey, whether it be a treatment plan or just regular preventive maintenance. Save time on your first visit by printing and filling out the new patient form before your first appointment.

For just a one-time annual payment, you and your family can receive your cleanings, x-rays, exams, as well as any needed emergency visits for the entire year, PLUS 20% off of any additionally needed treatment. Learn more.

This is hands down the best dental office I have ever been to. Dr.Nguyen is so gentle while she works and explains every step of the way. Her assistant Kim is patient and kind and makes your feel welcome. You are greeted at the front desk by Nathan who always has a smile on his face to welcome you.
